Your First Visit
Physical Therapy and Occupational Therapy
We are determined that each patient who visits Synergy Therapeutic Group will find it a pleasant, productive experience.
Our goal is to provide compassionate, evidence-based care to help reduce pain caused by injuries, surgeries or conditions, and restore your normal living.
We also strive to give this service within an efficient framework of clinic management that starts with the priority we place on scheduling your appointments.
Scheduling
As soon as your physician’s office sends us your prescription for care, we will contact you for an appointment within 24 hours. If you already have your physicians’ referral prescription and you call for an appointment yourself, you will receive one immediately.
If you wish merely to inquire about our services, you do have the option of scheduling a screening. A thorough evaluation and treatment cannot begin without a referral from your physician or specialist, however.
Before Your First Visit
If you have received the referral papers from your doctor, please bring them with you to your first appointment.
You should also bring all the information you have on your insurance or workman’s compensation so that our billing department has time to verify whether you will incur any out of pocket expenses. (All payments are due at the time of services rendered.)
It is also helpful if you print all the patient forms offered online in this section, fill them out, and bring them with you.
Please prepare a list of any prescribed or over-the-counter medication that you take regularly.
If your rehabilitation needs involve your lower body or back, please wear or bring shorts to your appointment. If it involves your upper body or back, please wear or bring a tank top, halter top or other appropriate clothing that will be comfortable for you to wear during your therapy. (Gowns are available for your privacy.)
If it is necessary to cancel an appointment, please contact Synergy Therapeutic Group as soon as you can to reschedule.
Your First Visit
Plan to arrive at least 30 minutes before your appointment to allow time for paperwork to be completed. If you have already downloaded and printed the patient forms from this website and filled them out, the process will be expedited.
We will be gathering information about the nature of your injury or condition, your medical history, any functional limitations and medications you are currently taking.
You must bring your insurance card or insurance information (claim number, adjuster’s name, etc. for workers’ compensation or auto insurance). Bring photo identification such as your driver’s license. Bring some form of payment and any referral or prescription from your physician.
What you can expect to happen at your first visit
During your first visit you will be seen one-on-one by a licensed physical therapist or occupational therapist.
Your condition will be evaluated and the source of your pain or physical injury determine. The evaluation process will include a thorough examination of muscle, nerve, tendon, ligament, bone and body tissues/organs as they pertain to your complaint or limitations.
Once your evaluation is completed, your therapist will develop a customized treatment program to address the problem. This includes physical deficits such as strength, posture, flexibility, neurological status, joint motion and functional limitation with activities of daily living.
Therapy treatment prescribed may include the use of manual therapy techniques, exercises and other kinds of manoeuvers which are performed with vestibular patients.
Therapeutic modalities may also be used to decrease swelling and pain, and/or to sooth injured muscles.
You will also receive instructions on what to do after the first treatment.
Your therapist will plan, implement and monitor your progress throughout the treatment program.
After Your First Visit
You may experience significant pain relief after your first visit. If you were previously unable to sleep through the night because of pain, you may likely sleep well following your first treatment.
The next day you may experience some soreness.
Follow the instructions you are given by your therapist. Do not miss any of your scheduled appointments.
The success of your treatment lies as much in your hands as others. The key is to keep up your appointment schedule and do whatever home exercises are prescribed.
