Subrat Bahinipati, PT, is the founder of Synergy Therapeutic Group in Carbondale, Illinois. A physical therapist with more than three decades of clinical experience, Subrat has built his practice around one principle that does not change: the body is not broken — it is adapting. His job is to find out exactly what it is adapting to.
His clinical focus is chronic pain, complex cases, and patients who have already been told by other providers that nothing more can be done. He sees patients who have tried injections, surgery, medication, and multiple rounds of physical therapy — and still hurt. His work is about finding the root pattern those treatments missed.
The Practice Story
In 2003, Subrat founded Synergy Therapeutic Group in Carbondale on a different idea about how physical therapy should work. The standard model of his industry — high-volume schedules, 15-minute slots, productivity quotas measured in billing units — was not the model he had trained for. So he built a clinic that operates on a single unbreakable standard: one therapist, one patient, one full hour, every visit.
Over the next two decades the practice grew into a multi-clinician group that includes adult physical therapy, pediatric occupational therapy under his wife and co-founder Chandana Dash, OTR/L, and a cash-based hybrid model that gives patients meaningful options outside insurance limits. The Synergy patient base now includes more than 230 five-star Google reviews and patients who travel from across Southern Illinois and beyond for care.
Clinical Approach
Subrat’s clinical work is rooted in a whole-body, root-cause approach. A patient who comes in with back pain does not get back-only treatment. The hip that stopped moving years ago, the fascia that pulled tight to compensate for an old injury, the nervous system that has been holding a protective pattern for so long it has forgotten how to let go — all of it gets attention.
His areas of focus include:
- Chronic and persistent pain — particularly cases that have not responded to standard care
- Complex musculoskeletal cases — multiple systems involved, long histories, mixed presentations
- Fibromyalgia and central sensitization — patients whose pain involves both tissue and nervous system layers
- Post-surgical recovery — including patients exploring whether surgery is necessary in the first place
- Manual therapy and myofascial release — hands-on work where it is indicated
- Pain neuroscience and patient education — teaching the system underneath the symptom

Outside of direct patient care, Subrat mentors physical therapists who want to build careers around outcome-driven clinical work rather than billing throughput. The Synergy mentorship model gives early-career clinicians the time, supervision, and business education they need to develop into clinicians who get results most of their peers cannot explain. Several Synergy alumni have gone on to build their own practices using the systems Subrat developed.
