THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our pledge regarding your health information
Synergy Therapeutic Group is required by federal law (the Health Insurance Portability and Accountability Act, “HIPAA”) to protect the privacy of your health information, give you this notice of our legal duties and privacy practices, and follow the terms of this notice currently in effect.
How we may use and disclose your health information
For treatment
We use your health information to provide you with physical therapy and occupational therapy services. This includes sharing information with other healthcare providers who are involved in your care — for example, your referring physician, your surgeon, or another therapist on our team.
For payment
We may use and disclose your health information to bill and collect payment for the services we provide — to you, to your insurance company, or to a third party responsible for your care.
For healthcare operations
We may use and disclose your health information to operate our practice — for quality assessment, staff training, business management, and similar administrative purposes.
Other permitted and required uses
We may also use or disclose your health information without your authorization for:
- Appointment reminders and treatment alternatives
- Health-related benefits and services that may be of interest to you
- Public health activities (disease reporting, FDA safety reporting)
- Reporting abuse, neglect, or domestic violence
- Health oversight activities (audits, investigations)
- Judicial and administrative proceedings (court order, subpoena)
- Law enforcement purposes when required by law
- Coroners, medical examiners, and funeral directors
- Organ and tissue donation
- Research (with appropriate safeguards)
- Serious threats to health or safety
- Specialized government functions (military, national security)
- Workers’ compensation claims as authorized by state law
Uses requiring your written authorization
Most other uses and disclosures of your health information require your written authorization. This includes — but is not limited to:
- Marketing communications
- Sale of your health information
- Most uses and disclosures of psychotherapy notes
- Use of testimonials, photos, or videos that identify you
If you authorize a use or disclosure, you may revoke that authorization in writing at any time, except to the extent we have already acted on it.
Your rights regarding your health information
- Right to inspect and copy. You have the right to inspect and obtain a copy of your health information, with limited exceptions. We may charge a reasonable fee for copying.
- Right to amend. You may request an amendment if you believe information in your record is incorrect or incomplete. We may deny your request in certain cases.
- Right to an accounting of disclosures. You have the right to receive a list of certain disclosures we have made of your health information.
- Right to request restrictions. You may request that we restrict certain uses or disclosures of your information. We are not required to agree to all restrictions but will follow any agreed-upon restriction.
- Right to request confidential communications. You may request that we contact you in a specific way (e.g. by phone at work only) or at a specific location.
- Right to be notified of a breach. If your unsecured health information is involved in a breach, we will notify you as required by law.
- Right to a paper copy of this notice. You may request a paper copy at any time, even if you agreed to receive it electronically.
Our duties
- We are required by law to maintain the privacy of your health information and to provide this notice of our legal duties and privacy practices.
- We must follow the terms of the notice currently in effect.
- We reserve the right to change this notice. A revised notice will be effective for all health information we maintain, including information received or created before the change.
- Any revised notice will be posted in our office and on our website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against in any way for filing a complaint.
