Scheduling your first appointment
When your physician’s office sends your prescription we will contact you for your appointment within 24 hrs.

If you are calling for your appointment and you have a physicians order you will be given appointment immediately.

If you are inquiring about our services, you have the option of scheduling a free screening that’s when we determine if we can help you. Your insurance company is not charged for this service.

Before Your First Visit

Please bring your referral if you received one from your physician.

Please give us all your insurance or workman’s compensation information so that our billing department has time to verify your benefits & Advise you if you have any out of pocket expenses. All payments are due at the time of service rendered.

You have the option of printing all forms online, filling them out, and bringing them in with you for your first visit.

If your rehabilitation is regarding your lower body or back, please wear or bring shorts to your appointment. Also consider bringing athletic shoes if pertinent to your condition.

If your rehabilitation is regarding your upper body or back, please wear or bring a tank top, halter top or other appropriate clothing which reveals the body part being treated. Gowns are available for your privacy and comfort.

Your first visit

Please plan to arrive 15 minutes before the start of your first appointment to complete new patient paperwork. You can also download the forms from this web site. This paperwork will include information about your injury, current functional limitations, and medical history.

You may want to adjust time(s) your spending with each patient – has changed –been reduced.

For your first visit:

• Bring your insurance card or insurance information (claim number, adjuster’s name etc. for work comp or auto insurance)
• Bring your photo identification card (e.g. driver’s license)
• Bring some form of payment (cash, check or credit card), for co-payment or deductible.
• Wear comfortable clothes
• If your physician requested physical therapy, please bring referral or prescription

Your physical\occupational therapy treatment

For every appointment at STG, you will be seen one-on-one by a licensed physical or occupational therapist or by a licensed physical \occupational therapist assistant. On your first visit, your therapist will evaluate your condition and identify the source of your pain or physical injury. Your initial evaluation will include a thorough examination of muscle, nerve, tendon, ligament, bone and body tissues\organs as they pertain to your physical complaints or limitations.

After your physical\occupational therapy evaluation, your therapist will develop a treatment program that will address physical deficits such as strength, posture, flexibility, neurological status, joint motion, and functional limitation with activities of daily living. Therapy treatment may include the use of manual therapy techniques, exercises and other kind of maneuvers which we perform for our vestibular patients. Therapeutic modalities may be used to decrease swelling, decrease pain and/or soothe injured muscles. You will also receive instruction what to do after first treatment. Therapist will plan, implement and monitor your progress throughout the treatment program.

The initial evaluation will typically last approximately 60 minutes. Subsequent appointments are scheduled for approximately 45- 60 minutes. Please arrive 5 minutes before each appointment. If you are unable to keep an appointment, we require at least 24 hours notice of cancellation.

After your first visit
You may find significant pain relief after your first visit. If you were unable to sleep through the night because of pain, you will likely sleep well following the first treatment.  The next day you might experience some soreness. Follow the instructions given to you at the time of evaluation. Do not miss any of your scheduled appointments. The whole success of our program is in your hands, the key to success is keeping regular appointments.
